documents and medical information at your fingertips. In the event that you
require immediate medical care, especially when being treated at an unfamiliar
hospital, you or your loved ones can simply present the attending physician
with your LegalVault wallet card. This card lists allergies, emergency contact
information and instructions on how to obtain all healthcare documents such as a
Health Care Power of Attorney, Living Will, HIPAA
Authorization and organ donation forms. Hospital personnel
will have limited access to your LegalVault information, allowing them to view
only your healthcare documents. By providing caregivers access
to your vital medical information, you can help prevent
hospital errors which occur all too often when treating new patients.

Business Compliance Service(TM) Program
We can provide business compliance solutions for private
companies -- insuring the necessary legal formalities are met,
including providing annual minute, service agent, and stock transfer
services to our clients.
For a single annual fee of $295 for each legal entity, we provide
companies with the following basic services:
- Prepare standard annual meeting minutes for shareholders and
directors (or for an LLC; members and managers) of the company.
- Act as the service agent for the company.
- Provide an annual company audit and compliance questionnaire
used to identify compliance issues and keeping
- Prepare a company profile reflecting the current owners and
management in addition to other key legal business items.
Additional services are provided at a reasonable cost to our
clients, including private company stock transfer services,
preparation of buy-sell (shareholder) agreements, and medical
reimbursement plans.
Of course, we also provide assistance in the formation of new
entities, including S Corporations, C Corporations, Limited
Liability Companies, and Partnerships.
